Who We Are

The Trilogy Community Foundation exists to improve the lives of Trilogy and Synchrony employees, future employees, and residents so that they may live their best lives. In 2021 the Foundation was able to assist over 1,500 employees through scholarships, emergency assistance, and the Susan’s Savings program. We also helped in funding some of our Live A Dreams so that our residents can experience their most memorable moments.

In 2022 the Foundation began funding a new program, Knock Out Hunger through Careshelf, to assist with food insecurity in our facilities. The Careshelf program is designed to provide additional resources to employees struggling to make ends meet and help ease some of the burdens.

We know that by serving employees, they will continue to exceed customer expectations, allowing Trilogy to be the Best Healthcare Company in the Midwest!
