Memorial or Tribute Gifts
A memorial or tribute gift is a thoughtful way to honor, remember, and celebrate the life of someone you love. Each campus or facility will have memorial envelopes and QR codes available for funeral homes, if you so desire. Often families will note the Trilogy Health Services Foundation in the funeral notice instead of flowers.
Please include “in honor of” or “memory of” the resident you wish to recognize in the memo line.

After you have contributed in memory or tribute, an acknowledgment letter will be sent to the family member, and a tax donation will be mailed to you. We do not disclose the contribution amount and are grateful for any amount you might choose to contribute.
